How to Obtain a Birth Certificate

How to obtain a Lost or Replacement PA Birth Certificate

Replacing a lost or replacement Pennsylvania birth certificate can be obtained at no or minimal cost depending on the category type of the application being used to obtain the birth certificate.  For details on ordering a PA birth certificate online visit the Pennsylvania Department of Health, website.

There are 4 Different Types of Applications Used to Apply for a Pennsylvania Birth Certificate.

1) Application for a Birth Certificate with Fees Waived for Individuals Who Are Experiencing Homelessness 


2) Application for a Birth Certificate with Fees Waived for an Individual affected by an Opioid Use Disorder


3) Application for a Birth Certificate with Fees Waived for Foster and Justice Involved Individuals


4) Application for a Birth Certificate with Fees Waived Under the Disaster Declaration for a Natural Disaster


All four application types are provided below. Follow the directions on the forms for next steps in the application process.
