Establishing Delaware County Residency

RESIDENCY POLICY

Purpose: Delaware County’s SCA shall provide treatment services to all eligible county residents in need. Eligibility is determined for individuals who are uninsured and or underinsured. The individual in the need of treatment services MUST be a Delaware County resident. The County Assistance Office (CAO) accepts county residency to be established in past 30 days, and the SCA will also accept this time frame. (Please note that while all efforts should be made to obtain proof of residency, the absence of proof will not prohibit an individual from receiving a Drug and Alcohol assessment or admission into a level of care)


How to Establish Residency· 

  • Obtain documentation verifying that the client is a resident of Delaware County
  • Documentation can be a either a photo ID or other accompanying documentation 

                (Please note, the absence of an ID will not prohibit an individual from receiving a Drug and Alcohol 

                assessment or admission into a level of care)· 

  • All documentation must be considered current and dated within 30 days of the requested date of service
  • Document must include a printed address


Acceptable Proof of Residency 

  • Current driver’s license or state identification card with address
  • Pay stub within past 30 days
  • Utility bills, with address, within the past 30 days (also will accept cable or credit card bills, not including letters announcing pre-approval for credit card, school loan bills and bank statements)
  • Notarized letter from landlord
  • Active renter’s lease agreement
  • Written communication from State agencies within 60 days 
  • Written court (State or County) communications within 60 days—NOT including District Judge’s summons, DJ appointment letter, or traffic tickets
  • Letter and accompanying envelope with clear postmark within 60 days
  • Passport issued or used within the past year
  • Copy of a police incident report 


Adolescents

  • Valid and current School ID
  • Notarized letter from family member, dated and signed, stating type and length of relationship


Homeless: Letter from shelter on letterhead with the exact information included in the Delaware County Adult Service Request for D&A Treatment Form
